Claim Forms
Death Certificate (Hospital and NADRA issued)
Attested CNIC of both member and claimant(s)
Complete medical/treatment record

Free Look Period
If you cancel within 14 working days of receiving your Participant’s Membership Document (PMD), you are entitled to a full refund of your contribution, less medical/clinical exam expenses
