How to Claim
In the unfortunate event of a claim, the beneficiary should contact 5th Pillar Family Takaful at 021-111-786-573 and provide:
- Original Policy Documents
- Claim Forms A, B, C, D
- Medical/Treatment Records with clinical notes
- Death Certificate (Hospital & NADRA issued)
- Attested CNIC copies (life assured & claimant)
- Written intimation with date and cause of death
For Accidental, Murder, or Suicide cases:
- Post Mortem / Autopsy Report (attested)
- Newspaper cutting (attested)
- Police FIR (attested)
- Medico-Legal Report (attested)
Note: Additional documents may be requested depending on the nature of the claim.
