Claims Process
To process a claim, please provide:
- Claimant’s Statement (downloadable at IGI Life claim forms)
- Physician’s Statement (downloadable)
- Original or attested Union Council Death Certificate
- Hospital death certificate
- Original Membership Document
- Medical treatment history (if applicable)
- CNIC copies of deceased & beneficiary
- FIR/Police Report & Post-Mortem Report (in case of accident)

Free Look Period
If you cancel within 14 days of receiving your Participant’s Membership Document (PMD), you are entitled to a full refund of your contribution, less medical/clinical exam expenses.
