How to Claim – Death Claim

Beneficiary should contact Jubilee Life at (021) 111-111-554 and submit:

• Original policy documents
• Completed claim forms (A, B, C, D)
• Complete medical/treatment records from onset of treatment
• Death certificate (Hospital & NADRA)
• Attested CNIC copies of life assured and claimant(s)
• Written intimation with date and cause of death

For Murder, Suicide, or Accidental Death:

• Attested post-mortem/autopsy report
• Attested newspaper clipping
• Attested Police FIR
• Attested medico-legal report
