How to Claim

1. Call Jubilee Life at (021) 111-111-554.
2. Submit:
   • Original policy documents
   • Claim Forms A, B, C, D
   • Complete medical/treatment records
   • Death certificate (hospital and NADRA)
   • Attested CNICs of life assured and claimant(s)
   • Written intimation stating date and cause of death

For murder, suicide, or accidental death, also provide:

• Post-mortem/autopsy report (attested)
• Newspaper cutting of the event (attested)
• FIR (attested)
• Medico-legal report (attested)

Further documents may be requested based on the case.
