Claims Process

In the event of a claim, the beneficiary should contact Jubilee Life at (021) 111-111-554 and provide:

• Original policy documents
• Completed claim forms (A, B, C, D)
• Complete medical/treatment record, including all clinical notes from the start of treatment
• Death certificate (Hospital & NADRA)
• Attested CNIC copies of both the life assured and claimant(s)
• Written intimation stating date and cause of death

For Murder, Suicide, or Accidental Death, also provide:

• Attested post-mortem/autopsy report
• Attested newspaper clipping of the event
• Attested Police FIR
• Attested medico-legal report
