Claims Process

To file a claim, beneficiaries must contact Jubilee Life at (021) 111-111-554 and submit:

• Original policy documents
• Completed Claim Forms (A, B, C, D)
• Medical and treatment records with clinical notes
• Death certificate (Hospital and NADRA)
• Attested CNICs of life assured and claimant(s)
• Written intimation of death, with cause and date

Additional documents for accidental, suicide, or murder cases:

• Post Mortem / Autopsy Report (Attested)
• Newspaper report of the event (Attested)
• FIR (Attested)
• Medico-Legal Report (Attested)
